Waiver understanding upon registration:  I/we, the parents of the above named registrants, hereby provide approval to participate in any and all FMYLA session activities.  I/we assume all risks incidental to such participation, and I/we release all liability and agree to hold harmless FMYLA, directors, supervisors, coaches, referees, and participants for any claim resulting from injury to my/our child, due to negligence or any other cause. In case of emergency, I/we hereby authorize 911 to be contacted for medical assistance.
